Replacement Diploma: this form and a $50 check must be submitted for someone who wants another copy of his/her diploma.
Academic Access: This form gives parents access to a student’s educational records. It must be signed by both student and parent.
Time Conflict: this form allows a student to register for two courses with overlapping times. It requires the signature of both professors and states how the conflict is to be resolved.
Senior Citizen Waiver: a form that allows senior citizens to take courses at reduced fees.
Registration Grid Sheet: a form that helps map out registration and allows a student to get a chair signature to register for a closed course.
Request to Exceed Normal Course Load: students who want to take more than 18 credits must fill out this form and get the signature of their academic adviser and dean.
Business Undergraduate Degree: form to be filled out by senior business majors in preparation for graduation.
Change of Personal Data Form: allows a student to change name, address, social security number and other personal information.
Honorable Dismissal: must be filled out by students who are withdrawing from the university.
Transcript Request: must be filled out and sent to the Transcript office.
Nursing Undergraduate Degree: form to be filled out by senior nursing majors in preparation for graduation.
Art & Science Undergraduate Degree: form to be filled out by senior majors in the College of Arts & Sciences in preparation for graduation.
Diplomacy Undergraduate Degree: form to be filled out by senior Diplomacy majors in preparation for graduation.
